About The Position

The Case Manager provides patient guidance and support, focused on social determinants of health and addressing barriers to care, and serves as an advocate for PrimeCare’s patients. The Case Manager is expected to carry out PrimeCare’s mission, vision, and values both internally and when representing the organization through external events or communications.

Requirements

  • One (1) to three (3) years of experience, preferred in social services. Background in a community health setting is highly preferred.
  • Ability to use MS Office programs, including MS Word, Excel and Outlook.
  • Bilingual in English/Spanish required.

Responsibilities

  • Serve as integral member of the care team: review daily schedules and patient charts, identify gaps in care and areas of need, schedule appointments, communicate needs and care plan with care team
  • Facilitate communication among patients, providers, nurses, medical assistants, and outside resources, related to the care of patients
  • Facilitate communication and serve as liaison between the health center and school staff by spending time in the school and connecting with CPS staff and students
  • Maintain timely and consistent communication and follow up with patients regarding their health care and resource needs
  • Conduct health risk assessments of patients and connect them to the appropriate community resources (insurance services, transportation, childcare, employment, legal, etc.) and provide consistent follow up to ensure services are received
  • Collaborate with site OB medical leadership, attending weekly case reviews on active OB patients, conduct outreach, and carry out all case management for the patient throughout pregnancy
  • Collaborate with Coordinator of School-Based Health on projects, outreach initiatives, tabling events, and other tasks as assigned
  • Perform population-based outreach to close gaps in care and ensure timely patient follow-up including, but not limited to, vaccine schedules, cervical cancer screening, Medicare Annual Wellness Visits, chronic condition follow-up, and prenatal care.
  • Participate in specialized programs for students and community members such as quality initiatives and workgroups, grant-funded programming, education programs, IBCCP coordination, etc.
  • Represent PrimeCare at community and school events, educating students and the community on our services and promoting our organization and programs
  • Ability to work occasional evenings and/or Saturdays for school outreach events
