Requirements

Bachelor's degree related to related fields, Social Work, Counseling, Psychology Criminal Justice, Health Education, Special Education OR Other Human Services Related Field.
For targeted case managers working with children have 1-year full-time experience working with children with behavioral health concerns.
For targeted case managers working with adults have 1-year full time experience working with adults with behavioral health concerns.
Targeted Case Managers required to bill Medicaid.
For targeted case managers (CBHCM) Certified Behavioral Health Case Manager certification must be obtained through the Florida Certification Board (FCB) within one year of employment.

Responsibilities

Provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those whose involvement in the child welfare system requires.
Demonstrate competency in the domains of Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility.
Coordinates the service delivery for individuals and provides community partners with up-to-date available referral resources to provide continuity of care.
Effectively maintain and manage their assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
Possess a clear understanding of the service delivery system in accordance with regulations and district policy.
Provide services at the time and location that is most appropriate to the person served.
Travel is required.
May also be required to conduct urine drug screening and transport clients, depending on the program.
Some programs may also require a 24 hour on-call rotation.
Perform other duties as assigned.
