Adapt Integrated Health Care is looking for a qualified Case Manager to join their Behavioral Health team! This position works Monday-Friday. The Behavioral Health Case Manager works with individuals diagnosed with mental illness to promote recovery, independence, and successful community living. This is accomplished through the provision a variety of treatment services including crisis intervention under the guidance of a therapist (QMHP) and/or supervisor, individual and group skills training, and development; case management services; coordination and monitoring daily structured activities of clients. Performs crisis intervention as necessary and provides consultation and training. Facilitates communication and correspondence with other Adapt staff, community partners, other treatment providers and peers. Position coordinates and collaborates with multi-disciplinary treatment teams and is responsible for work performed in assigned specialized program area (i.e., ACT, EASA, PSRB, .370, Jail Diversion, etc.).
