Responsible for providing, coordinating and monitoring a variety of Case Management Services to a designated caseload of Mental Health consumers. Caseload may be any combination of Mental Health consumers ranging from pediatric, adolescent, adult and/or geriatric ages. Under the supervision of the Clinical Manager, Case Manager is responsible for providing, coordinating and monitoring services for their respective caseloads, eight hours per day, five days per week. Employee is subject to on call by the Department as designated by the needs of the agency. ESSENTIAL JOB FUNCTIONS: Completes a comprehensive assessment of the consumer’s strengths, current status and goals to ensure necessary services are provided. Develops an action plan with the consumer to obtain service goals. Provides crisis assistance in situations where immediate attention is warranted. Provides daily living assistance to help the consumer achieve service plan goals and enhance their ability to live independently or in a home with others. Provides linkage, referral and advocacy and services to help the consumer access and mobilize resources to meet their needs. Completes required records, forms and other documentation according to agency guidelines. Meets or exceeds agency standards related to productivity and timely completion of paperwork. Have daily interactions with clients who are mentally ill, seriously emotionally disturbed and/or chemically dependent who, due to their impairment, are at high risk for labile, volatile and/or unpredictable behavior. Performs related responsibilities as required or directed. Participates in QA&I, Utilization Management and other agency functions. Adheres to Case Management Guidelines/Standards as noted by TennCareStandards.
