About The Position

The Opportunity: We are seeking to hire a Case Manager to join one of our regional treatment teams. This individual enhances the provision of clinical services to clients by offering practical help, building community support, teaching recovery-based skills, and monitoring services.

Requirements

  • Minimum of an Associate's Degree in a human service field required.
  • Prior experience in case management working with individuals with severe and persistent mental illness preferred.
  • A car and a valid Ohio driver's license and insurability through the Agency's insurance carrier required.
  • A deeply held belief in CSS's recovery-centered mission.
  • Ability to develop positive relationships with individuals with mental illness, developmental disabilities, and substance abuse challenges.
  • Skill in assessing individuals for strengths, needs, abilities, and preferences.
  • Excellent organizational, attention to detail and communication skills.
  • Ability to work in a team environment.
  • Proficiency in Microsoft Outlook.
  • Skill in managing time and meeting deadlines.
  • Ability to negotiate assertively from an advocacy perspective.
  • Knowledge of community resources and demonstrated ability to access.

Responsibilities

  • Serve on a team built up of fellow Case Managers, Recovery Specialists, and medical staff to navigate the needs and care of your clients, attending team meetings on a regular basis.
  • Monitor and coordinate the delivery of necessary evaluations and assessments in order to identify each person's needs.
  • Assist individuals to identify and minimize the effects of their psychiatric symptoms, enabling individuals to self-manage their illness.
  • Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives.
  • Connect clients with community resources to help them develop interpersonal and community coping skills.
  • Assist the individual in fostering and increasing their social support networks of relatives, friends, and/or volunteers.
  • Accompany individuals to appointments and activities, assisting in daily living skills training as needed.
  • Maintain relevant documentation.
