The Case Management Assistant at Baystate Medical Center plays a crucial role in supporting the Case Management team by ensuring effective communication and documentation of clinical and regulatory information. This position requires collaboration with patients and Case Managers to facilitate a seamless flow of information, which is essential for the delivery of quality care. The Case Management Assistant will be responsible for auditing charts to ensure compliance with Medicare requirements and effectively communicating relevant regulations to the team. This role also involves assisting with follow-up appointments and managing telephone calls related to post-acute and transitional care needs, ensuring that patients receive the necessary support during their recovery process. In addition to direct patient interaction, the Case Management Assistant will coordinate with various internal and external partners, including insurance companies, accountable care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies. This collaboration is vital for addressing financial advocacy for patients and ensuring that all aspects of their care are managed efficiently. The Case Management Assistant will be a key player in the overall care coordination process, helping to bridge gaps between different service providers and ensuring that patients have access to the resources they need. This full-time position requires availability during standard business hours, specifically from 8:00 AM to 4:30 PM, Monday through Friday, with a rotation for weekends and holidays. The Case Management Assistant will be based at Baystate Medical Center, located at 759 Chestnut Street, Springfield, and will be part of a dynamic team dedicated to providing high-quality healthcare services to the community.
