LCMHA (Lenawee Community Mental Health Authority) provides or manages a full array of services to adults and children with mental health and/or emotional disturbances, persons with substance use disorders, and persons with developmental disabilities. Our mission is to create a path to resilience, recovery, wellness, and self-determination for these individuals. The Case Manager Assistant – Home-Based (Children's Services) assists in meeting the needs of consumers receiving case management services. Specifically, this individual will: Provide supports and services to a consumer as identified in the child's Individual Plan of Service Perform duties under guidance of the Home-Based clinician, which may include assisting and training in: Assisting family implementation of behavioral interventions; Assisting family is practicing pro-social behavior; Supporting parents in implementing parenting strategies identified in plan; Modeling positive parent/child interactions; Application for benefits. Maintain documentation of contacts with consumers Assist the team in maintaining the calendar of consumer contacts Participate in Home-Based team meetings and huddles to ensure cohesive interventions with families served Provide other assistance to families as delegated by the Home-Based team Regular and consistent attendance as assigned Carry out additional duties as assigned by supervisor.
