About The Position

The Case Manager - Active at Developmental Pathways (DP) is responsible for managing a caseload of individuals receiving Waiver and State General Fund services. This role involves conducting assessments to determine eligibility for services, maintaining documentation, and advocating for individual service needs. The Case Manager will facilitate interdisciplinary team meetings to coordinate services and ensure effective communication among team members. This position offers a hybrid work environment, allowing for both remote and in-person work.

Requirements

  • Bachelor's Degree; or
  • Five (5) years of experience in Long-Term Services and Supports (LTSS), including Developmental Disabilities; or
  • A combination of education and relevant experience appropriate to the position requirements.
  • Valid Colorado motor vehicle license and proof of motor vehicle insurance.
  • Meet driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years.

Nice To Haves

  • One (1) year of experience in developmental disabilities/long-term services and supports.

Responsibilities

  • Manage a caseload of individuals receiving Waiver and State General Fund services.
  • Complete assessments to determine eligibility for services/supports.
  • Identify individual strengths, needs, concerns, and preferences through interviews and standardized tools.
  • Maintain documentation needed quarterly and annually.
  • Facilitate interdisciplinary team meetings to coordinate services.
  • Advocate for individual service needs and ensure the effectiveness of services delivered.
  • Provide case management services and support to temporarily assigned individuals.
  • Support with department and team coverage as needed.
  • Ensure timely entry and distribution of required documentation to team members.
  • Coordinate support in emergency and crisis situations.
