About The Position

Assertive Community Treatment (ACT) program is to develop and provide high quality, community-focused behavioral health services to improve the everyday lives of those who may be living with serious and persistent mental illnesses or conditions.

Requirements

  • Relevant bachelor’s degree (i.e. in social work, human services, or related field).
  • 1 years minimum of directly relevant experience in the Mental Health Field, Substance Use Disorder Field.
  • Experience with mild to severe mental illness and substance use disorders.
  • Experience in coordinating services with other agencies.
  • Equivalent combination of education and experience may be evaluated and considered.
  • Includes driving a company vehicle and requires a valid Colorado driver’s license. At times, personal vehicle may need to be used (mileage on personal vehicle use is reimbursed).

Nice To Haves

  • Bi-lingual / bi-cultural.
  • Previous multi-disciplinary team experience.
  • CAS and/or ability to obtain CAS credential preferred.

Responsibilities

  • Coordinates a wide variety of services for clients to receive benefits from internal and external service providers regarding housing, social security, etc.
  • Provides assistance and coaching to client/consumers with daily life activities such as bill paying/managing finances, expense tracking, completing applications for services, and other needs which may include acting as client’s Social Security payee.
  • Educates consumer/client on behaviors to maintain daily life activities.
  • Interacts regularly with federal, state and local entities to create opportunities to deliver services to client/consumer. Represents clients/consumers with these entities to get services, amend services, or reinstate services.
  • Perform case management services as needed to assist clients to meet their goals as stated in their service/treatment plans. Refer clients/consumers to supportive services such as vocational, medical, social security, housing authority, etc.
  • Assist consumers in improving their well-being and quality of life by coaching in behavioral skills; educate consumers to make informed choices; develop clients coping skills; help consumers to problem solve and negotiate tasks of daily living.
  • Documents progress reports, case management notes, letters, faxes and phone calls. Maintains all required paperwork for payee ships.
  • Maintain close and cooperative relationships with community agencies and other public agencies and private business such as landlords and banks in carrying out supportive services.
  • Advocate for consumers in many different forums including at the federal, state and local court systems, with family, school system etc.
  • Perform other duties as assigned.
