POSITION SUMMARY: Provides ACT case management services in the community to consumers with severe mental illness and their families. Assists in planning, linking, advocacy, coordination, and monitoring; to assist consumers in gaining access to health services, financial assistance, employment, education, social services and natural supports. Assesses needs, develops, and monitors Person Centered Plans and documents treatment in an accurate and timely manner. Provides video or audio assessments after hours on a rotated schedule between all team members.
