About The Position

The Department of Economic Security (DES) strengthens individuals, families, and communities for a better quality of life in Arizona. DES is seeking individuals committed to service, community, and teamwork. The Division of Developmental Disabilities (DDD) within DES is looking for an experienced and highly motivated Case Manager 3 for Support Coordination. This role involves providing flexible, high-quality, and member-driven services and supports to individuals with developmental disabilities and their families, empowering them to make independent decisions and engage in the community. Applicants must not be licensed or certified by the DDD Office of Licensing Certification and Regulation, have a principal interest in or a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, nor be employed or subcontracted by a DDD Qualified Vendor.

Requirements

  • Knowledge of methods, procedures and techniques used in the development of Individual Support Plans.
  • Knowledge of Federal and state statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities.
  • Knowledge of principles and practices of case management.
  • Knowledge of community resources, medical and behavioral health issues and terminology.
  • Skill in written and oral communication to convey a wide variety of information.
  • Skill in initiating, developing and maintaining member service plans.
  • Skill in interviewing.
  • Skill in counseling and interacting with members and family members.
  • Ability to maintain confidentiality.
  • Ability to work independently and within diverse work teams.
  • Ability to lead a team of Support Coordinators in the absence of the supervisor.
  • Candidate must be an Arizona licensed Registered Nurse in good standing OR possess a Bachelor Degree in Social Work (SW), OR Psychology, Special Education or Counseling AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental illness (SMI); OR candidate must have a minimum of two (2) years’ experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental illness (SMI).
  • A Bachelor’s Degree in Human Service, Behavioral Science or related field AND have at least one (1) year experience providing case management services to persons who are elderly and/or persons with physical or developmental disabilities and/or members determined to have a Serious Mental illness (SMI) OR have at least three (3) years’ experience working with or for individuals with intellectual or developmental disabilities.
  • Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-1968 in order to work with children and vulnerable adults.
  • Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
  • Successfully pass background and reference checks; employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions.
  • If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then Driver’s License Requirements apply.
  • All newly hired State employees are subject to and must successfully complete the Electronic Employment Eligibility Verification Program (E-Verify).

Responsibilities

  • Drive vehicle to conduct on-site visits to members/families’ homes to develop relationships, become familiar with family dynamics/needs/supports, and make appropriate referrals to community agencies, in compliance with requirements.
  • Convene and facilitate multi-disciplinary Individual Support Plan meetings to determine and identify services.
  • Provide assistance and support to members/families in crisis.
  • Attend meetings with members/families, service providers, and others as required.
  • Compile information from a variety of sources to make recommendations for eligibility determinations and re-determinations of eligibility.
  • Write comprehensive progress notes and maintain case records.
  • Prepare a variety of reports and complete special projects.
  • Attend staffing meetings, training, and other meetings (including statewide travel).
  • Confer with supervisor.
  • Act as Supervisor in Supervisor’s absence.
