About The Position

The Early Childhood Service Coordinator at Lutheran Services Florida is responsible for providing case management services to families with children aged birth to 6. This role involves maintaining case ownership for at least 30 open cases, coordinating services and supports, and ensuring the well-being of children and families. The coordinator will engage with families, conduct home visits, develop individualized support plans, and provide timely referrals to necessary services. The position requires flexibility in working hours, including evenings and Saturdays, to meet the needs of the families served.

Requirements

  • Bachelor's Degree in Social Work, Early Childhood Education, or a related field required.
  • Experience in case management for at least 30 open cases with families who have children ages birth to 6.
  • Bilingual in English/Spanish preferred.

Responsibilities

  • Provide case management services for at least 30 open cases with families who have children ages birth to 6.
  • Plan early childhood events at each Center to engage families and refer to St. Joseph's Hospital for developmental screenings.
  • Conduct home visits to meet family needs and ensure successful enrollment in key services.
  • Work with parents/caregivers to develop individualized family support plans.
  • Provide families with timely referrals to services.
  • Partner with families to build strong parent-child relationships and strengthen their support networks.
  • Maintain regular contact with service providers and document service progress.
  • Provide community education through public presentations.
  • Organize, prioritize, and complete all work assignments by established deadlines.
  • Develop and maintain knowledge of community resources and program eligibility requirements.
  • Work collaboratively with volunteers and community resources.
  • Accurately document all case activities and maintain case files in accordance with standardized formats.
  • Attend all appointments, staff meetings, trainings, and workshops as required.
  • Participate in Center events and support contractual outcomes.
