About The Position

Participates as a leader of case management staff involved in care coordination and discharge planning activities to proactively support efforts that ensure safe, effective, high-quality, and efficient patient care coordination on a day-to-day basis.

Requirements

  • Five years of broad clinical experience, predominantly in pediatric care
  • Graduation from an accredited school of nursing
  • Bachelor's degree (nursing or other related field)
  • Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact
  • Case Management Society of America, American Case Management Association, American Nurses Credentialing Center, or equivalent certification within 12 months of eligibility
  • Excellent communication skills, both verbal and written
  • Effective decision making/problem-solving skills and creativity in problem solving influential leadership skill
  • Demonstrated effective critical thinking skills and ability to anticipate patient discharge needs
  • Moderate to expert computer skills
  • Working knowledge of financial aspects of third-party payors and reimbursement
  • Must be able to successfully pass the Basic Windows Skill Assessment at 80% or higher rating within 30 days of employment

  • Experience in care coordination, case management, discharge planning and/or utilization review
  • Previous leadership or management experience preferred

Responsibilities

  • Manages day-to-day coordination of clinical staff involved in care coordination and discharge planning activities and takes daily assignments.
  • Acts as resource for case managers and practitioners and assists with problem solving and difficult cases.
  • Assists with training and orienting new employees regarding their role in care coordination and quality management as well as appropriate policies/procedures.
  • Coordinates and assists with ongoing physician education related to care coordination.
  • Maintains a relationship with other departments and appropriate personnel to facilitate inpatient care coordination process and provide continuity of care.
  • Maintains knowledge of current relevant state and federal regulations, Joint Commission standards and criteria, and documentation requirements.
  • Assists in coordinating department training activities.
  • Assists in development and revision of guidelines, pathways, and protocols.
  • Assists in determining staffing requirements and levels to ensure that inpatient care coordination services are rendered in the most effective and efficient manner.
  • Assists in facilitating care coordination staff meetings along with director and/or manager.
  • Serves as acting manager in absence of manager for problem solving and as a informational resource as needed.
  • Assists in maintaining departmental budget within established fiscal guidelines.
  • Assists with performance evaluations and provides input into hiring and disciplinary actions.
  • Performs other responsibilities as required.
