JOIN OUR TEAM AND MAKE A DIFFERENCE!“Action Inc is a non-profit organization dedicated to developing pathways to economic stability in Southwest Montana through innovative, community-based, collaborative approaches.” Case Management Team LeadEmployment/FLSA Status: Full-Time/Exempt POSITION SUMMARY: The Case Management Team Lead plays an integral role in the delivery and administration of housing and case management services for the youth and adult populations served by Action Inc. This position is instrumental in deploying the agency approach to housing case management, providing leadership and direction to a team of case managers and ensuring case management services are delivered with fidelity to Action Inc. sanctioned approaches. The Team Lead works within the framework of the Continuum of Care system, providing integrated services that help create pathways to permanent housing. This position is responsible for maintaining a community Coordinated Entry Team and ensuring current, accurate data is maintained. It is responsible for providing guidance and technical assistance to a team of well-informed, effective team members who deliver trauma-informed, well-executed services including housing navigation, rent assistance, system navigation, landlord relations, advocacy, education and training, and tenancy supports. The Team Lead will maintain a case load as needed. The position monitors services and files for compliance with federal rules and completes related reports and fully understands grants and associated contracts that fund the case management team. The Team Lead is responsible for creating a team culture that aligns with the agency’s core values, is expected to model the core values of Action Inc. and maintains a standard of excellence in service delivery that aligns with the agency’s mission. SUPERVISION: The Case Management Team Lead supervises a team of up to 7 case managers.

Requirements

  • The knowledge and abilities are usually acquired through advanced education in a Human Services field and five years’ related experience with supervision and work with disadvantaged populations is preferred. Other combinations of education and experience which could provide this knowledge, or these skills and abilities, will be evaluated on an individual basis.
  • Knowledge of evidence-based approaches addressing homelessness, youth stabilization, and system development is expected. Knowledge of trauma-informed care and “Housing First” are required and can be acquired through training.
  • Ability to learn and effectively use software and data systems related to program implementation and proficient with Microsoft Office and Windows-based applications.
  • Strong attention to detail and a data-driven approach to work.
  • Ability to lead a highly motivated, effective team of case managers and maintain workplace culture in alignment with agency core values.
  • Excellent interpersonal, written, and verbal communication skills.
  • Must have the ability to deploy trauma-informed approaches.
  • Must possess the ability to work with staff, funders, clients, and other agencies in a consistently courteous and positive, professional manner.
  • Must be bondable and insurable.
  • Must possess a valid driver’s license.
