Requirements

• CPR & First Aid Certification, TB Clearance.
• Valid Hawaii driver’s license.
• Holds a Bachelor’s degree in Psychology, Social Work, or a related field.

• Demonstrates exceptional problem-solving and crisis management skills, along with a commitment to ethical practices.

Responsibilities

• Provide case management support to up to ten individuals, fostering their journey through coordinated treatment and care.
• Facilitate access to psychiatric evaluations, medication management, and essential health services.
• Enhance team effectiveness by assisting the Team Lead with organizational tasks, compliance tracking, and staff training initiatives.
• Engage in our Quality Improvement Committee and offer vital after-hours support to ensure continuous care.
