About The Position

The Case Manager Supervisor drives the delivery of exceptional services by the Case Management team, ensuring alignment with the highest professional standards, contractual obligations, and the core mission and values of IMPACCT Brooklyn. This leadership role is crucial in directing the Case Management team and managing program staff across two supportive housing facilities. The ideal candidate will possess robust clinical and organizational skills, as well as excellent verbal and written communication abilities, complemented by essential computer proficiency. They will effectively collaborate with multiple agencies while overseeing daily program operations. Importantly, the Case Management Supervisor approaches persistent mental illness with a strengths-based, hopeful perspective and is fully committed to maintaining the highest level of programmatic excellence.

Requirements

  • LMSW or Master’s Degree in Social Work with the expectation that LMSW is acquired within 6 months of hire, or a Master’s Degree in Mental Health Counseling with License (LMHC) or NYS equivalent degree, with 3 years’ experience.
  • Experience working with diverse client populations, including older adults and/or individuals living with behavioral health and mental health challenges.
  • 2 years of supervisory experience. Residential setting experience preferred.
  • 2+ years of related experience in the development and facilitation of substance use and harm reduction models, training, psychoeducation, and individual and group work.
  • Understanding of the Housing First model and person-centered, trauma-informed, and solutions-focused methodology.
  • Strong leadership and interpersonal skills.
  • Ability to work independently and effectively in a fast-paced environment.
  • Strong written and verbal communication skills required.
  • Proficient computer skills, including experience with MS Office Software.
  • Fluency in the English language is required.
  • Must be willing to work a flexible schedule.

Nice To Haves

  • Bilingual in Spanish/Mandarin/Cantonese is a plus.

Responsibilities

  • Supervision of direct reports, internal and external reporting, and ensuring the quality of supportive services, community outreach, screening, assessment, and eligibility reviews.
  • Lead clients through the intake and assessment process to provide orientation to the program and to identify service needs.
  • Develop a mutually agreed-upon service plan with clients to address needs and attain desired outcomes.
  • Develop and maintain positive linkages with clients, internal and external agency services, generic community resources, and state/city liaisons.
  • Monitor client success through participation in case conferences, staff meetings, reviews of program plans, and development of discharge plans.
  • Document case management activities in a confidential, timely, and concise manner.
  • Responsible for completing annual psychosocial assessments for all clients in the program and conducting reassessments.
  • Maintain a strong working relationship with the social service staff to ensure due diligence and recommend best practices.
  • Actively participate in interdisciplinary case conferencing.
  • Provide crisis intervention as needed.
  • Provide substance abuse case conferencing, training, and consultation to the social service team.
  • On-site supervision of Gibb Mansion Residence and Lexington Senior Residence.
  • Work with direct reports to develop an annual calendar of recreational, socialization, educational, and skill-building activities with a view towards client inclusion and integration.
  • Work with the Director to coordinate and supervise daily operations.
  • Assist in developing and implementing plans and goals for the Social Service Department.
  • Assist with budgeting and monitoring expenses.
  • Ensure that Case Management services are delivered with quality care and documented in compliance with professional standards of case management, the contractual requirements of the funding sources, and agency standards.
  • Participate in job-related training.
