About The Position

The Kentucky Department for Community Based Services (DCBS) is a multi-faceted agency that provides services and program to enhance the self-sufficiency of families; improve safety and permanency for children and vulnerable adults; and, engage families and community partners in a collaborative decision-making process. DCBS is searching for a (Adult Medicaid) Case Management Specialist I who is highly skilled and motivated to help others. This includes providing professional services directly to the public in reviewing, monitoring and determining continued eligibility for assistance programs. The Case Management Specialist I works with families to assess their needs in order to achieve self-sufficiency. Attention to detail, experience communicating with the public and the ability to track deadlines are all skills necessary for this position. The Case Management Specialist I is a demanding and rewarding position that requires leadership, maturity and a strong knowledge of policy. Career growth is encouraged through training and mentoring. Requirements

  • Graduate of a college or university with a bachelor's degree.

Responsibilities

  • Interviews clients and applicants for Medicaid and related programs including eligibility determination.
  • Maintains contact with participants and monitors effectiveness of goals.
  • Schedules and conducts need assessments based on participants history, current situation or screen for risk factors and barriers.
  • Refers to job ready participants to other agencies specializing in placement activities.
  • Develops participant transitional assistance plans for families based on their needs and capabilities.
  • Identifies resources and makes appropriate referrals while coordinating services.
  • Completes forms and gathers required documentation to determine eligibility
