About The Position

The Case Management Program Supervisor is responsible for overseeing the DAS Case Management program and providing case management services to seniors and adults with disabilities. This role includes assisting the Supervising Care Manager (SCM) in overseeing the Case Management Program and Short-Term Home Care for Older Adults. The supervisor will train, supervise, and evaluate the program’s staff while providing essential case management services.

Requirements

  • Bachelor’s degree in health, social work, counseling, psychology, gerontology, or related fields with extensive case management experience and some administrative/supervisory experience.
  • Master's degree is strongly preferred.
  • Knowledge, skills, and experience necessary to assess the consumer’s needs for services and to perform the core functions of case management.
  • Sensitivity to the needs of the elderly and disabled adults from diverse ethnic and minority cultures.
  • Excellent communication and interpersonal skills.
  • Organized and able to multitask.
  • Able to work independently and in a team setting.
  • Bilingual and bi-literate in English and Chinese.
  • Must be able to travel via public transportation or personal vehicle for job duties.

Responsibilities

  • Assist SCM to oversee the Case Management Program and Short-Term Home Care for Older Adults program with DAS.
  • Train, supervise, and evaluate the program’s staff.
  • Provide case management services to seniors and adults with disabilities, including conducting intake, completing comprehensive assessments, developing care plans, implementing care plans, following up and monitoring plans, reassessing client needs, and terminating case management services when appropriate.
  • Maintain complete client files and complete all required data collection forms and reports.
  • Participate in evaluation and quality assurance activities to determine the effectiveness of the Social Services programs.
  • Assume responsibility for own professional growth and continuing education to upgrade case management skills.
  • Participate in case conferences, in-service training, and other meetings as required.
  • Network with and conduct outreach to community-based organizations, other service providers, and families and caregivers.
  • Attend and make presentations at community meetings to inform the community about the agency’s services.
  • Perform other duties as assigned.
