About The Position

The Case Management Nurse at Prime Dx is pivotal in managing patient care and ensuring optimal outcomes through effective coordination of health services. This role involves assessing, planning, implementing, monitoring, and evaluating options and services to meet an individual's health needs through communication and available resources.

Requirements

• Associates Degree in Nursing (ADN) required, Bachelor’s preferred.
• Valid and active RN license
• Minimum of 3 years of clinical nursing experience, with at least 2 years in case management.
• Strong clinical skills with a comprehensive knowledge of the principles and practices of healthcare management.
• Experience with oncology and transplant management
• Excellent communication and interpersonal skills, with the ability to manage sensitive information.
• Strong organizational and time management skills.
• Ability to work independently and as part of a team.
• Proficiency in medical software and Microsoft Office.

Responsibilities

• Assess new and ongoing cases, determining appropriate care and management plans.
• Coordinate with healthcare providers, patients, and families to implement care plans effectively.
• Monitor patient progress and adjust care plans as necessary.
• Facilitate communication between patients, family members, and healthcare professionals.
• Advocate for patient needs and ensure that they receive appropriate and timely medical care.
• Document and maintain accurate records of patient care, condition, progress, and concerns.
• Performs other duties as assigned.
