About The Position
The Case Management Liaison works directly with unit Nurse Case Manager(s) and Social Service Specialist(s) and reports to the Manager of Nurse Case Management & Nurse Navigation. The Case Management Liaison supports the Case Management team to assist with the completion of required forms/documentation, coordination of care, and post-acute follow-up arrangements. The Case Management Liaison collaborates with the Case Management team and other members of the interdisciplinary team to ensure that patients receive exceptional care and avoid unnecessary delays in discharge. This position does not directly manage any other caregivers.
