About The Position
This is a telework position with a 50-75% travel requirement to meet members face to face. The candidate must live in the Northside of Chicago/Evanston. As an Analyst, Case Manager, you will facilitate appropriate healthcare outcomes for members by providing care coordination, support, and education for members through the use of care management tools and resources. You will conduct comprehensive evaluations of referred members' needs and eligibility, recommend approaches to case resolution, and coordinate care plan activities while monitoring progress. You will also consult with various stakeholders to overcome barriers to care and ensure quality of care.
