We are seeking a Case Management Assistant for our team to ensure that we continue to provide all patients with high quality, efficient care. Did you get into our industry for these reasons? The Case Management Assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.
