About The Position

The Case Management Assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative clerical support in Case Management functions.

Requirements

  • Associate Degree preferred.
  • Medical knowledge and terminology is required.
  • Minimum of one year experience in a health care setting is required.
  • C.N.A license preferred.
  • Possess the ability to work Monday - Friday, 8a - 4:30p and rotating weekends.

Responsibilities

  • Coordinate with RN case manager and social worker for support needed.
  • Communicate updates on progress throughout the day.
  • Set up transportation via the Transfer Center and print discharge packets for post-acute care transfers.
  • Use web-based referral system to manage post-acute care referrals.
  • Prioritize workload to ensure a timely contact and intervention with PAC providers.
  • Monitor patients with post-acute care referrals and intervene to expedite discharge.
  • Communicate with patient and have the Patient Choice letter signed.
  • Deliver the Import Medicare Message letter to the patient per facility policy.
