How you’ll make an impact in this role Champion Patient Transitions: Expedite the discharge process by ensuring all clinical documentation is seamlessly transmitted, reducing wait times and helping patients move to the next stage of their recovery faster. Bridge the Communication Gap: Act as the vital link between clinical teams, admissions, and insurance payers to ensure everyone is aligned on the patient’s status and care plan. Secure Care Access: Manage the authorization process for ancillary services and handle denial reporting to ensure patients receive the specialized treatments they need without administrative delay. Empower Patient Choice: Facilitate and document all external referrals with a focus on honoring patient preferences, ensuring they transition to facilities that best meet their personal and medical needs. Coordinate Safe Transfers: Partner closely with Case Management to arrange secure patient transport and transfers, maintaining constant communication to ensure no detail is missed during the hand-off.

Requirements

  • Provide various services supporting case management process.
  • Obtain and records authorizations for ancillary services and denial reporting.
  • Facilitate, identify and document all referrals, along with patient choice, to appropriate facilities or agencies.
  • Expedite discharges by transmitting appropriate documentation to providers for acceptance of patient. Document all interactions with payers and communicate status with appropriate staff.
  • Communicate status with case management staff and arranges for patient transfer. Function as a key point of contact between case management staff, admissions and payers.
