POSITION SUMMARY: The Case Management (CM) Assistant works collaboratively with Case Management and the Treatment Team to support timely patient progression to the appropriate level of post-hospital care throughout the patient’s hospital stay. Early in the patient’s hospital stay, the CM Assistant will interview the patient, caregiver, and/or decision-maker to document the patient’s baseline functioning in the pre-hospital environment and complete the discharge evaluation as required by Centers for Medicare & Medicaid Services (CMS). Through the patient’s hospital stay, the CM Assistant interacts directly with patients, caregivers, and/or decision-makers and gathers pertinent patient information for continued care transition needs while providing excellent patient experience. Ensures all communication is appropriate, accurate, timely, and consistent with the patient’s plan of care.
