About The Position

Under the leadership of the Director of Case Management or the Care Management Leadership team, the Care Management assistant supports the departmental functions to include, but not limited to, organization of workflow, communications, critical information tracking, report creation and community resources. This position is focused on supporting the Care Management department in optimizing the Care Management processes.

Requirements

  • High school diploma.
  • 1 year clerical experience.
  • Data entry skills.
  • Demonstrable skills with Google Docs.
  • Demonstrable skills with Google Sheets.
  • Proficiency in Microsoft Suite.
  • Familiarity with email applications.

Responsibilities

  • Support the departmental functions including organization of workflow.
  • Facilitate communications within the department.
  • Track critical information.
  • Create reports.
  • Assist in managing community resources.
