Position Summary
The Case Coordinator provides organizational and programmatic support, including admissions assistance, direct participant support, care coordination, and communication with families, staff, and external partners.

Requirements

• Bachelor’s degree in a Social Services–related field required
• Two years of experience supporting individuals with disabilities in a medical or rehabilitation setting
• Ability to travel as needed

Responsibilities

• Support and coordinate services to ensure participant needs are met.
• Assist participants with understanding their goals and programs.
• Serve as a liaison for families, providing updates and resources.
• Organize and lead participant conferences.
• Review evaluations, progress notes, and discharge summaries.
• Maintain accurate participant documentation.
• Assist with discharge planning and follow-up.
• Communicate case-specific updates with team members.
• Attend required meetings, including clinical meetings.
• Review monthly bills and report discrepancies.
• Coordinate participant incentive accounts with the Case Manager.
• Perform other duties as assigned.
