About The Position

The Family Case Aide will work in a child welfare prevention program that utilizes the Solution Based Casework model to stabilize families with children under 18 years old who are at risk for placement outside of their homes. Preventive Services aim to stabilize and provide support so that children can remain in their homes. Requirements

  • High school Diploma or equivalent required
  • Child welfare experience.
  • Must be willing to do home visits.
  • Good verbal and written communication skills.
  • Ability to learn the SBC model to partner with families to establish Action Plans and assist them to reach their goals.

Responsibilities

  • Conduct weekly home visits
  • Develop client assessments and service plans following the model outlined by improved outcomes for children, Family Team Conferences and Solution Base Practice
  • Maintain case records, service plan (FASP) and knowledge of Connections and PROMIS.
  • Participate in weekly supervision and case conferences.
  • Participate in trainings which will enhance and develop skills needed to carry out the task for Children Services and Hearthsare St, Vincent Services.
