Day Shift - 7.5 Hours (United States of America) The Case Aide provides supportive services to consumers and families while assisting with care coordination, documentation, and collaboration with internal and external partners to ensure continuity and quality of care. Completes intake evaluations as scheduled, makes appropriate referrals, reschedules cancelled intake appointments, and conducts and documents outreach attempts when appointments are not kept. Provides individualized and group-based services tailored to the needs of consumers and families, including case conferencing, coordination with families, and collaboration with outside agencies, programs, and service providers. Completes program notes, initial intakes, service plans, substance use screenings, updates, assessments, and discharge summaries in a timely manner in accordance with program procedures. Develops and maintains knowledge of hospital-based and community-based resources and refers consumers and families to appropriate services to support continuity of care. Communicates effectively with interdisciplinary staff to ensure collaboration and integration of educational and vocational goals and participates in scheduled staff meetings. Engages in ongoing professional development through supervision, peer supervision, conferences, and mandatory in-service trainings to enhance knowledge, cultural competence, and self-awareness. Adheres to hospital-wide and departmental policies and procedures, including those related to infection control, confidentiality, patient rights, safety, disaster protocols, and fire prevention. Works toward meeting departmental productivity goals for monthly visits and collaborates with supervisory staff to address barriers to maintaining a full caseload. Maintains accurate, timely, and complete documentation of services provided, including progress notes, monthly statistical reports, and other required records. Performs additional duties as assigned.

Requirements

  • B.A./B.S. degree required, preferably with a concentration in a social or human services field
  • Valid driver’s license and daily use of a personal vehicle required
