Redwood Coast PACE with Humboldt Senior Resource Center is looking for a full-time CarePartner to provide care for our participants in our Day Center in Fortuna. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health. The schedule for this position is Monday through Friday, 8:00 am to 4:30 pm. JOB SUMMARY: Under the supervision of the Therapeutic Services Coordinator, the CarePartner provides personal care and support services to Redwood Coast PACE participants in the Day Center. This role is responsible for assisting with activities of daily living, monitoring health status, supporting therapeutic and nutritional needs, and helping to maintain a safe, clean, and engaging environment. The CarePartner plays an essential role in fostering participant independence, dignity, and well-being while ensuring care is delivered in accordance with each participant’s individualized Plan of Care, departmental guidelines, and regulatory standards. Effective communication, teamwork, and compassion are key to success in this position.
