About The Position

The Family Caregiver Support Specialist will work under the WellMed Charitable Foundation, the philanthropic partner of WellMed Medical Management and will implement and direct all aspects of Caregiver SOS by WellMed. Program elements include telephonic dementia support, caregiver education, information and referral services, education & training, resource navigation, care coordination, and program development. This role focuses on empowering caregivers through education, care planning and behavioral support, delivered in person and remotely.

Requirements

  • Bachelor’s Degree or equivalent experience
  • 4+ years of experience in a social services environment
  • Intermediate knowledge of Microsoft Office suite (including Word, Excel, and PowerPoint)

  • Bilingual Spanish/English
  • Experience in dementia care, care coordination, case management and or caregiver support
  • Solid communication and interpersonal skills, especially in a telephonic setting
  • Ability to work independently and manage a caseload remotely
  • Ability to exercise judgment and discretion in handling sensitive and confidential issues

Responsibilities

  • Receive and manage referrals from primary care providers, community and senior centers for patients diagnosed with cognitive or other chronic conditions.
  • Provide ongoing telephonic education on dementia progression, behavioral symptoms, and caregiving strategies tailored to the caregiver’s needs.
  • Utilize established software platforms (TCARE) to conduct required caregiver assessments.
  • Develop and update individualized care plans that reflect the goals and preferences of the caregiver.
  • Providing caregiver support and guidance by connecting families with essential community resources, including respite care services and advance care planning.
  • Provide regular follow-up as needed to activate informal and formal resources and caregiver’s support according to the caregiver’s care plan.
  • Maintain accurate and timely documentation of all interactions, assessments, and care plans in compliance with program protocols and HIPAA regulations.
  • Educate and train caregivers on how to become active members of the care team, including strategies for managing dementia-related behaviors.
  • Facilitate virtual/online support groups, educational dementia series/workshops as well as facilitate evidence based 9-week Stress Busting Program.
  • Complete all required and ongoing training to maintain compliance and ensure up-to-date knowledge and skills.
  • Performs all other related duties as assigned
