About The Position

Join Our Team as a Caregiver Support Coordinator! Are you passionate about supporting the elderly population, and making a real difference in the nonprofit sector? EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 100 impactful programs across Long Island and NYC, is looking for a full time Caregiver Support Coordinator to join the National Family Caregiver Support program. Salary: $28 to 32 per hour Schedule: Monday-Friday, 35 hours per week with flexibility for evening caregiver groups / workshops Location: 99 Quentin Roosevelt Blvd in Garden City What drives the core purpose of this job: The Caregiver Support Coordinator delivers caregiver education and support services and facilitates access to respite services under the National Family Caregiver Support Program. The position provides caregiver coaching and support while coordinating access to services in alignment with program requirements.

Requirements

  • Master’s Degree in Social Work required; LMSW preferred.
  • Experience working with caregivers, older adults, or in human services.
  • Experience facilitating groups and delivering trainings preferred.
  • Strong communication, engagement, and interpersonal skills.
  • Strong organizational and coordination skills.
  • Proficiency in Microsoft Office and database systems.
  • Must have valid New York State Driver’s License and have access to an automobile.

  • Bilingual, Spanish Speaking preferred.

Responsibilities

  • Develop and deliver caregiver education workshops on caregiving skills, stress management, and disease specific care.
  • Facilitate caregiver support groups and provide individual caregiver coaching and support.
  • Conduct outreach to recruit caregivers into education, support, and respite services.
  • Coordinate referrals to EAC in-home respite services and confirm linkage to services.
  • Track and monitor authorized respite services to ensure services are delivered as planned.
  • Coordinate alternative respite services, including external providers, when needed.
  • Serve as primary point of contact for caregivers regarding respite access and service issues.
  • Maintain accurate records of caregiver education, support services, and respite utilization.
  • Collaborate with Caregiver Case Managers to align services with individualized service plans.
  • Coordinate with in-home respite program staff to address service gaps or issues.
  • Develop partnerships with community providers to expand respite options when needed.
  • Assist with program reporting related to caregiver education, support services, and respite utilization.
  • Ensure compliance with all EAC Network policies and applicable funding and regulatory requirements.
