About The Position

Care Transitions Nurse- WEEKENDS Fri/Sat/Sun Department: 10172 Advocate Aurora Health Corporate - WI Transitional Care Status: Full time Benefits Eligible: Yes Hours Per Week: 36 Schedule Details/Additional Information: 0.8 FTE Weekend position Friday-1130-2000, Saturday & Sundays 0800-2030. Fully remote position, High speed internet is a requirement. Occasional holidays Must hold a WI OR IL RN license (will be required to hold both state license after hire). Pay Range $35.50 - $53.25 Requires WI or IL RN license at time of application Requires WI or IL residency Requires BSN or in route to complete ADN to BSN program Responsible for ensuring an efficient and coordinated hospital discharge and transition process for patients recently hospitalized or treated in the emergency room that are at high risk for increased use of healthcare resources.
