About The Position

Joining our Virtual Care Center (VCC) Department, we are currently recruiting for a Care Transitions Associate to join our team. The care transitions associate will join the VCC care coordination team to ensure patients who have had an ER visit or hospital stay are provided with an immediate follow up appointment at one of our facilities. Patients will be followed for thirty (30) days with the Care Transitions Associate providing timely contact with patients. A Care Transition Associate is essential to support this expanded volume, ensuring timely patient outreach, documentation, discharge coordination, and completion of transitional care activities.

Requirements

  • HS Diploma is required, Associates or Bachelors Degree in Medical Administration or related field of study is preferred.
  • 2 years of experience working in a community mental health or human services setting.
  • Bilingual English/Spanish required.
  • Excellent oral and written communication skills.
  • Ability to serve professionally and manage time and duties well.
  • Familiarity with care coordination efforts and liaising between patients, providers, and staff.

Responsibilities

  • Conduct timely outreach to patients following hospital or emergency department discharge
  • Confirm discharge instructions, medications, follow-up appointments, and understanding of care plans
  • Identify barriers to care such as transportation, medication access, housing, or caregiver support
  • Collect, monitor, and report transitions of care data, including patient contact success rates, follow-up appts
  • Coordinate communication between patients, primary care providers, specialists, and care team members
  • Conducts follow up for admitted patients a minimum of two times within a 30 day time frame.
  • Refers patients to Asthma Manager if the patient was hospitalized or in the ER due to Asthma or to other related programs in the organization based on condition.
  • Orients the patient and coordinates the initial primary care follow-up visit and reminds/encourages the patient.
