About The Position

The Care Management Assistant is responsible for helping patients access and navigate services and resources. The Care Management Assistant provides support to the department through an integrated approach. Activity will promote, maintain, and improve set-and-agreed-upon departmental goals.

Requirements

• High School Diploma/GED
• 1 year of health care experience with patient exposure (hospital, home care, skilled nursing facility)

Nice To Haves

• College degree in health, human services or related field
• Teaching or presenting information to groups
• Experience with EHR and Microsoft Office

Responsibilities

• Provide Direct Services
• Promote wellness by providing culturally appropriate health information to providers, patients, and/or community members
• Provide individualized care support
• Navigates work utilizing EHR and telephonic communication within the system.
• Provides support to the work processes for the Care team
• Navigate the health and human services system
• Increase access to care through intentional and targeted communication
• Make appropriate care team connections and coordinate services
• Teach people the knowledge and skills needed to obtain care
• Facilitate continuity of care
• Link patients to and inform them of available resources
