About The Position

Overview Coordinates and directs the referral activities from health care facilities to home care by identifying/determining appropriateness of care, gathering medical and financial data, meeting with patients and families, and working with care management, physicians and referral sources to ensure a smooth transition. Responsible for safe and effective transfer from facility to home, and securing patient referrals by maintaining customer relationships.

Requirements

  • High School Diploma (Required)
  • Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI

  • 1 year of home care, case management or discharge planning experience (Required).
