As a Patient Care Team Nurse, you will continuously evaluate patient care systems, collaborate with the healthcare team to ensure continuity of patient care, and report patient progress. This position will work closely with the medical staff and will report to the Site Operations Director. The role involves participating in continuous quality improvement activities, promoting effective communication among healthcare team members, performing patient assessments and data collection, conducting pre-visit planning, identifying patient problems, maintaining care coordination records, initiating and documenting patient and family teaching, identifying unsafe patient environments, discussing emergency plans with Pediatricians and Nurse Practitioners, maintaining open communication with medical staff, assisting patients and families in functioning at their optimum level, regularly meeting with Providers and other Patient Centered Medical Home team members to identify clinical problems, assisting in implementing continuous quality improvement programs, coordinating services for patients and families utilizing other health related agencies, obtaining referrals, following approved protocols, and ensuring efficient patient flow.
