As a Care Team Lead, you will guide a dedicated team to ensure high-quality patient care, compliance, and operational excellence. This role combines leadership, training, and quality assurance to make a meaningful difference every day.

Requirements

  • Experience in home health care, patient navigation, 0r care coordination strongly preferred
  • 1+ years in a Care Team or Patient Navigation role is preferred
  • 1+ years of supervisory experience preferred
  • Key competencies include delegation, critical thinking, problem solving, and leadership

Responsibilities

  • Lead and inspire care team members to achieve excellence in patient care
  • Set clear expectations, goals, and priorities for team members, aligning efforts with organizational objectives
  • Handle escalated patient concerns promptly and professionally, ensuring resolution and maintaining trust in care delivery
  • Design and deliver impactful training programs that elevate team performance
  • Provide ongoing guidance and support to enhance professional development and competency
  • Ensure all documentation meets regulatory and organizational standards through rigorous quality checks
  • Complete regular audits and implement quality assurance measures to maintain compliance
  • Conduct patient visits to ensure care standards are being met
  • Collaborate with the Care Team Director to establish and track performance metrics
  • Monitor progress, identify areas for improvement, and implement strategies to achieve targets
  • Foster a culture of collaboration and teamwork within the care team and across departments
  • Facilitate effective communication and coordination to ensure seamless patient care delivery
