The Care Support Coordinator is responsible for proactive and/or retroactive patient scheduling. Primary duties may include but are not limited to: scheduling patients so that home-based provider availability is maximized and to ensure that patients are seen at the right time. Location: Virtual - This role enables associates to work virtually full-time, with the exception of required in-person training sessions (when indicated), providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. How you will make an impact: · Provide support to providers by answering emails and inbound call transfers. · Support the transition of care process. · Assist providers and patients with HEDIS gap closures and HRA compliance. Associates in this role are expected to have the ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers.
