Requirements

  • One (1) year of experience as a Medical Assistant, Paramedic, EMT, Military Allied Health Professional, Nursing Assistant, or similar healthcare role within value-based care or physician offices.
  • Strong understanding of medical terminology and patient engagement.
  • Excellent communication, critical thinking, and problem-solving skills.
  • Ability to work both independently and as part of a multi-disciplinary team.

Responsibilities

  • Support patients transitioning between sites of care, ensuring their needs are met from hospital to home.
  • Collaborate with clinical and administrative teams across the organization.
  • Help develop post-acute care plans and perform routine patient check-ins.
  • Distribute resources that remove care barriers and prevent avoidable readmissions.
  • Advocate for patients by identifying needs such as prescriptions, equipment, and follow-up services.
  • Maintain strong proficiency with organizational systems, including EMR tools.
  • Contribute to a psychologically safe environment where ideas are encouraged and valued.
