Overview The Care Management Specialist promotes positive patient and family experience by partnering with the health care team, external agencies, and payers to support care coordination during admission. The role, based on assignment, includes serving as a liaison between internal teams, external agencies/facilities, patients, and caregivers to provide exceptional care coordination for successful discharge. Coordinating patient progression activities during the patient's hospital course, supporting utilization management functions, and administrative functions for the Care Management Department. The Care Management Specialist fosters collaboration to positively impact patient safety and care quality. The Care Management Specialist works in partnership with the health care team to support coordination of care to improve patient care and safety; Assists in facilitating interdisciplinary communications during inpatient care. The position also provides administrative support to the Care Management department, UR, RN, and SW Care Managers, and assigned resident team. This includes accountability for delegated non-licensed activities such as scheduling appointments, obtaining signed referral documents, arranging inpatient studies and procedures, follow-up appointments and facilitation of communication with community agencies and PCP offices, creating and updating databases, preparing and disseminating reports, and ordering supplies and equipment.
