About The Position

The Care Manager will assure that Health Home enrollees receive needed medical, behavioral, and social services with the goal of improving individual health outcomes. Will have overall responsibility and accountability for coordinating all aspects of the individual’s care. Will collaborate with multidisciplinary personnel (i.e. medical, mental health, nurses, etc.) to assure patients receive services as indicated in the enrollee’s plan of care.

Requirements

• Bachelor’s degree in Health, Human Services, Social Work, Mental Health, or qualified field required or Associate’s Degree in Health, Human Services, Social Work, Mental Health or certification as an RN or LPN and two years’ qualifying experience.

Responsibilities

• Coordinating all aspects of the individual’s care
• Collaborating with multidisciplinary personnel to assure patients receive services as indicated in the enrollee’s plan of care
