About The Position

The Connecticut Hospice (“CT HOSPICE”) operates a 52-bed inpatient hospital, a palliative care outpatient program and a home health agency for care of patients/families facing irreversible illness. CT Hospice has recently been selected by the Centers for Medicare and Medicaid to participate in the Guiding an Improved Dementia Experience (GUIDE) model of care. As a GUIDE participating provider, CTH will coordinate care, caregiver support services and respite care for People Living With Dementia (PLWD) to enable them to remain at home. POSITION SUMMARY: CT HOSPICE is seeking an experienced individual capable of understanding the needs of Persons Living with Dementia (PLWD) and their caregivers. The ideal candidate is bilingual English/Spanish and has experience with case management/care coordination.

Requirements

  • Must be able to build strong relationships and work collaboratively with all levels of individuals within various agencies, CT Hospice departments, partner organizations, and other stakeholders.
  • Able to communicate information clearly and succinctly with CMS, community partners, stakeholders, and individuals from the community effectively, clearly, and positively via phone, email, and in-person.
  • Must be highly detail-oriented and have excellent organizational and time management skills.
  • Ability to multi-task, manage, and prioritize various projects, assignments, and grants as well as work effectively toward deadlines.
  • Must be able to work both as a team player and independently in a frequently changing environment.
  • Must possess and maintain a valid Driver’s License and insurance and must successfully complete a motor vehicle history check.
  • This position requires driving a personal vehicle on behalf of CT HOSPICE, travel (up to 30%) may be required.
  • Bachelor’s degree in an appropriate field preferred, (social work, public health, health administration, or other health related field) and/or relevant experience.
  • Three to five years of demonstrated skills managing programs, supporting care coordination/patient navigation staff, conducting community outreach and interfacing with community partners.
  • CT Community Health worker certified and/or willing to obtain.

Responsibilities

  • Assists with the ongoing execution of the GUIDE model with focus on recruitment of Medicare beneficiaries with dementia diagnoses through community outreach and collaboration with home health agencies and assisted living facilities throughout New Haven, Fairfield and Middlesex counties.
  • Promotes the use of telehealth and HIT for care navigation and caregiver support integral to the GUIDE model.
  • Assists with the implementation of an annual GUIDE program workplan that represents alignment with GUIDE model requirements.
  • Participates in compilation of data in order to ensure accurate monitoring and timely completion of program goals and CMS requirements.
  • Collaborates with GUIDE Team members and CT HOSPICE’s Finance and Billing departments to maintain compliance with the GUIDE participation agreement with CMS and ensures timely submission of narratives, data, and budget reports to CMS for the GUIDE program.
  • Performs other department or agency-related duties or special projects as assigned.
