About The Position

The Enhanced Care Management (ECM) Care Manager supports individuals and families in addressing their health and care management needs. The Care Manager uses independent judgment and discretion to provide direct services including case planning, advocacy, outreach, and home visitation activities for children and families in collaboration with a team. Requirements

2+ years experience providing case management in an intensive level program.
Bachelor’s degree in Social Work, Health, Public Health, or a related field with relevant work experience
Background in health care services and/or public health
Possess the ability to engage and connect with diverse population including but not limited to individuals experiencing homeless, at-risk for homelessness, and under-resourced communities, persons with mental health and substance abuse disabilities
Comfortable with conducting outreach virtually, in-person, and by phone, including making cold calls
Detailed oriented – can complete the necessary paperwork (intake forms, Health Action Plan, etc.)
Available to work evenings or weekends, as needed
Has knowledge of and possesses strong engagement skills as well as crisis and de-escalation skills to provide quality care and services to clients and families.
Reliable transportation
Possesses a valid driver's license and state-required auto insurance.
Ability to handle confidential information.
Strong interpersonal skills and critical thinking abilities
Able to work independently and take initiative
Establish and maintain effective working relationships with those contacted in the performance of required duties
Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.
Sensitivity to service population’s cultural and socioeconomic characteristics.
Excellent verbal and written communication skills.
Punctual and dependable attendance.
Must have intermediate knowledge of computer programs in a Windows environment.

Proficient in Electronic Health Records System

Responsibilities

Outreach to potential members from a call list (MIF), inform them of the program and its benefits, and enroll them in services. Outreach includes phone calls, in-person visits, letters, emails and text messages.
Complete CII’s intake forms and a comprehensive assessment, care plan, and Health Risk Assessment (HRA)
Ensure goals of individuals from care plan are being met
Serve a liaison between member, healthcare team, and other community resources.
Link family to relevant and requested community resources
Assist member and their support system with health literacy
Provide health promotion and self-management training.
Advocate on behalf of member with healthcare professionals and community programs.
Identify community resources for member via internet research, community directories, and/or their fellow staff members
Accompanies ECM members to office visits, as needed and according to applicable guidelines.
Monitor treatment adherence and informs teams of any changes or lack of adherence.
Arrange transportation as needed
Ensures all client files are accurately and completely maintained.
Establishes and maintains rapport and effective working relationships with children and families and works effectively within CII and social work community at large.
Uses independent judgment and discretion to identify client’s needs and provide community referrals to client and others.
Advocates on behalf of the client with other agencies, government programs and similar to help them receive needed services.
Ensures accurate and timely completion of outcome and evaluation testing and reports required by the program.
Travels extensively within LA County and on occasion, beyond LA County.
Conducts intake interviews to gather information and completes required paperwork.
Attends educational workshops reviewing professional publications, establishing personal networks, benchmarking state-of-the art practices, and participating in in-service programs.
Develops and maintains knowledge of service standards and policies as stipulated by contract, licensing, and/or other governing bodies.
Provides transportation to clients based on needs to various appointments.
Other duties and special projects as assigned.
