About The Position

As a Care Manager, you will work within our positive parenting program called Triple P located in Cherry Hill, Camden County NJ. The Triple P Care Manager will work alongside the Practitioners, throughout service delivery, when families are experiencing challenges which impact safety and connection of the family unit. The Care Manager will connect with families directly and act as a resource broker. They will build relationships with families and the community; acquiring knowledge of community based resources, and linking families to those supports that will enhance their protective factors. Positive Parenting Program (Triple P) Overview: The Triple P System is a suite of interventions that enhances the knowledge, skills and confidence of parents and caregivers of youth. Triple P gives parents clear strategies to respond to their child’s needs and remain calm, promoting attachment as children learn and meet developmental milestones. Triple P encourages parents to create a nurturing environment, allowing children to grow in a safe, and loving home. Triple P encourages self-regulation, the capacity to solve problems independently, and reduces dependency on others. Schedule: Full-time; Monday-Friday with 2 evenings per week; Weekends as needed

Requirements

  • Bachelor’s degree in Psychology, Social Work, or equivalent experience within the child welfare system preferred.
  • Minimum of at least one years’ experience in parent/family support, maternal-child health
  • Candidates should also demonstrate knowledge and experience in care coordination for families and community stakeholders
  • Valid Driver's License in good standing
  • All positions require a valid driver’s license in good standing and pre-employment drug screening.

Responsibilities

  • Provide concrete support to families that are receiving Triple P services.
  • Identify gaps and barriers to services, advocating for families as needed.
  • Provide on-going assessment of families by engaging and partnering with parents throughout the course of program participation.
  • Refer families to services in a timely manner, prioritizing those based on severity of needs
  • Ensure the successful linkage to community resources by providing transportation, if needed.
  • Collaborate with DCP&P and other referral partners to connect families with community resources to meet family needs.
  • Develop and maintain relationships with a diverse array of community programs to effectively meet the needs of individual families.
  • Collaborate with Practitioners and Supervisor to identify immediate service needs or resources for families.
  • Participate in advisory councils/boards within their local community/area of service to gather beneficial information to assist in supporting families.
  • Participate in agency activities, required meetings, and trainings as required by agency and/or DCF.
  • Complete the Positive Parenting Program training and other trainings required by agency and/or DCF.
  • Regularly research and document community resources including services related to health, housing, education, employment, and social supports.
  • Share any updated resource information with the Practitioner and Supervisor.
  • Complete required documentation, forms, and produce statistical reports.
  • Participate in quality improvement and evaluation activities to enhance service delivery.
