Care Manager
About The Position
Castor Home Health Solutions is dedicated to delivering high-quality, compassionate care in the home setting. We are seeking a motivated Registered Nurse (RN) to join our team as a Care Manager. This leadership role is primarily field-based and focuses on overseeing patient care, supporting and training staff, and ensuring clinical excellence. You will play a key role in maintaining high standards of care while building strong connections with both clients and caregivers. A company vehicle is provided for travel to patient homes.
Requirements
- Current Registered Nurse (RN) license in Illinois
- Minimum 1 year of experience in home health and pediatrics
- Strong clinical judgment and leadership skills
- Excellent communication and interpersonal abilities
- Knowledge of healthcare regulations and compliance standards
- Ability to work independently and as part of a team
- Valid driver’s license and clean driving record
Responsibilities
- Conduct patient admissions and ongoing supervisory visits
- Train, mentor, and support field staff
- Develop and oversee patient care plans and physician orders
- Provide direct patient care as needed
- Ensure compliance with healthcare regulations and company policies
- Build and maintain strong relationships with clients and caregivers
- Collaborate with the Director of Nursing (DON) on clinical and office-related tasks
