This role ensures appropriate level of care, facilitates transitions, and promotes efficient throughput while maintaining compliance with regulatory standards.
Requirements
- Graduate of an approved school of nursing
- Current Florida RN license and BLS certification (or within 90 days of hire)
- Minimum 3 years in chronic disease management, care coordination, utilization management, or acute clinical care
- Strong communication, problem-solving, and organizational skills
- Ability to work in a fast-paced environment
Responsibilities
- Assess patients for medical necessity, care coordination, and discharge planning
- Assign initial DRG and monitor length of stay using InterQual guidelines
- Lead multidisciplinary care conferences and complex case reviews
- Communicate with patients, families, and interdisciplinary teams regarding care plans
- Ensure compliance with federal, state, and Joint Commission standards
- Manage concurrent disputes and prevent avoidable days
- Collaborate with physicians, nursing, and other departments to optimize care delivery
