This role ensures appropriate level of care, facilitates transitions, and promotes efficient throughput while maintaining compliance with regulatory standards.

Requirements

  • Graduate of an approved school of nursing
  • Current Florida RN license and BLS certification (or within 90 days of hire)
  • Minimum 3 years in chronic disease management, care coordination, utilization management, or acute clinical care
  • Strong communication, problem-solving, and organizational skills
  • Ability to work in a fast-paced environment

Responsibilities

  • Assess patients for medical necessity, care coordination, and discharge planning
  • Assign initial DRG and monitor length of stay using InterQual guidelines
  • Lead multidisciplinary care conferences and complex case reviews
  • Communicate with patients, families, and interdisciplinary teams regarding care plans
  • Ensure compliance with federal, state, and Joint Commission standards
  • Manage concurrent disputes and prevent avoidable days
  • Collaborate with physicians, nursing, and other departments to optimize care delivery
