About The Position

To provide leadership, support and supervision to Care Managers. This includes ensuring that the standards and requirements of the Care Coordination Organization/ IDD Health Home are met including delivery of the 6 core areas of Health Home requirements. Supervisors must measure productivity in all skill areas, ensure billing and regulatory compliance and track and provide training and development in the delivery of Health Home care management.

Requirements

  • Bachelor’s degree with two years of relevant experience, or
  • A Master’s degree with one year of relevant experience, or
  • License as a Registered Nurse with two years of relevant experience.
  • A valid Driver’s License
  • Must be able to work a flexible schedule and use personal vehicle for business travel purposes.
  • Must be willing to participate in rotating on-call responsibilities
  • Exceptional strategic thinking and leadership skills and promotes active engagement of employees
  • Effective resource and fiscal management
  • Duties require professional verbal and written communication skills.
  • Proficiency in or knowledge of using a variety of computer software and e-mail applications, especially Microsoft Excel, Outlook and Word; have the aptitude to learn other computer software as necessary.
  • Completion of CQL interviewer accreditation within one year of hire date.

Responsibilities

  • Maintain a small caseload of members as necessary
  • Provide oversight and support to a team of Care Coordination Managers
  • Review and approval of Life Plans ensuring that all regulatory and internal requirements have beenmet.
  • Knowledge of IDD/ Behavioral Health and Health Care systems and best practice both locally and on anational level.
  • Ensure regulatory compliance in accordance with oversight agencies and internal CCO policies.
  • Provide individual and group supervision of Care Coordination Managers.
  • Participation in workgroups to assist with review and implementation of program policies andprocedures.
  • Tracking productivity through performance management and quality metrics.
  • Oversight of Care Coordination Managers caseloads and caseload acuity; assignment of new membersand coordination of disenrollment
  • Enforce progressive discipline as necessary
  • Negotiating with service providers and community based organizations to ensure the highest quality ofservice is being provided to enrollees.
  • Assist with audits and surveys required for maintaining CCO designation and improvement.
