About The Position

Care Manager - Middlesex County - Part time - Hourly rate $43.00 - The areas highest quality behavioral healthcare providing care management services to clients which includes assessing and assisting with their needs and coordinating care in a warm and compassionate environment, 8 +/- hours per week, 9:00am - 5:00pm Monday - Friday - The Care Manager will follow through and assist clients with their plan of care, and coordinate care services ensuring that those we serve receive the highest quality of service in a caring and compassionate atmosphere. Senior Care Options by Parker: Senior Care Options by Parker provides customized Geriatric Care Management services as well as Behavioral Health services by licensed social workers to individuals in their homes and communities across Middlesex, Monmouth, and Ocean Counties. Program participants of both programs benefit from services offered in their own environment by our dedicated care partners who are committed to meeting their needs. Services will help support participants in maintaining independence in their own environment.

Requirements

Bachelor's degree required
Ability to read, write, understand, and communicate effectively in English
NJ drivers license in good standing
Ability to deal with others in a diplomatic, tactful and confidential manner
Ability to maintain composure under stress or in dealing with residents, staff, and families
Flu vaccine required during flu season
