Care Manager RN - Care Management Per Diem On-Call Day
About The Position
Care Manager RN - Care Management at Providence St. Vincent Medical Center. This is a Per Diem/On-Call, Day Shift position, with 8-Hour Shifts from 800-1630. RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care. Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for Care Management (CM) services. Requirements
- Nursing degree/diploma required upon hire.
- Bachelor's Degree: Bachelor's degree in nursing or higher within 3 years of hire.
- upon hire: Oregon Registered Nurse License
- 2 years of acute care hospital or community-based experience.
- 1 year of experience with electronic medical records or documentation systems.
- 1 year of home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.
